Exit Pass (Filipina maid)

Please bring the maid and the following documents along:

1. Valid Philippines contract
2. Valid OWWA
3. A copy of employer's NRIC / Passport copy
4. A copy of FDW's Work Permit card
5. A copy of the Air ticket to hometown
6. S000 Philippines Embassy Bond
7. OFW information sheet (form is available in our office.)

       *Fee: SGD 50.00

 

Renewal of Passport (Filipino maid)

Please bring the maid and the following documents:

1. Philippine Standard Contract (to be signed by employer and FDW)
2. OWWA membership (to be signed by FDW)
3. SGD 2000 Philippines Embassy bond (to be signed by Employer)
4. Philippines passport application
5. FDW's original passport
6. A copy of FDW's work permit card
7. 3 copies of maid's photos (passport size, white background, with collar)
8. A copy of employer's NRIC / passport for expatriate

* For numbers 1-4, forms are available in our office.
    Fee: SGD 380.00

 

Renewal of Work Permit (Filipino & Indonesian maid)

Please bring the following documents:

1. SGD 5000 Security bond and SGD 15,000 personal accident insurance (to be signed by employer)
2. Security bond form (to be signed by employer)
3. Renewal form from MOM (to be signed by both employer and FDW)
4. Maid's original passport
5. Maid's original Work Permit card

* For numbers 1 and 2, forms are available in our office.
    Fee: SGD 120.00 (excluding insurance)

 

Home Leave Processing (Filipino maid only)

Please bring the maid and the following documents along:

1. Philippine Standard Contract (to be signed by employer and FDW)
2. OWWA membership (to be signed by FDW)
3. OFW information sheet (to be signed by FDW)
4. SGD 2000 Philippines embassy bond (to be signed by employer)
5. Copy of present insurance of FDW
6. A copy of the Air ticket
7. FDW's original passport
8. A copy of FDW's work permit card
9. A copy of employer's NRIC / Passport copy for expatriate
